Streamlining the Process

As part of the high level of service we provide to our patients, we are respectful of your time and strive to streamline the intake process. We are now operating with electronic medical records. Every patient will need to be registered into our system. In order to make your appointment as seamless as possible, below we have provided printable links to the intake documents you will be asked to complete before your visit. Simply click on the document to print. Complete the paperwork ahead of time and remember to bring with you to your appointment.

We also ask that you come to your appointment 15 minutes early. This time will allow us to handle the administrative part of your visit with us.
